Abstract [sv]
Forskningsprojektet ”Fysisk aktivitet för hälsosamma hjärnfunktioner bland skolungdomar”, förkortat ”Hjärnhälsa i skolan”, syftar till att undersöka förhållanden mellan vanemässig fysisk aktivitet och hälsosamma hjärnfunktioner (dvs goda tankemässiga funktioner och god psykisk hälsa), de akuta neurofysiologiska effekterna av fysisk aktivitet, stillasittande och kost, samt hur skolor kan bedriva fysisk aktivitet för främjande av hälsosamma hjärnfunktioner.

Kärnfrågan i projektet lyder: Vilket är förhållandet mellan fysisk aktivitet, stillasittande, kost och hälsosamma hjärnfunktioner bland skolungdomar, och hur kan fysisk aktivitet för främjande av hjärnhälsa på att långsiktigt hållbart sätt genomföras i skolor?

Bakgrunden till forskningsfrågan står att finna i den växande psykiska ohälsan och sänkta skolresultat bland barn och ungdomar i Sverige och de flesta andra länder i västvärlden. Psykisk ohälsa betyder inte bara ökat lidande utan också sämre förmåga att studera och arbeta och en högre risk för framtida sjukdomar. Nedsatta kognitiva förmågor kan påverka möjligheten att klara akademisk utbildning och utgör därför ett hot mot svensk konkurrenskraft i ett globalt perspektiv. Projektet ”Hjärnhälsa i skolan” fångar upp flera av de globala målen för hållbar utveckling i Agenda 2030 och bidrar till att förvandla målen till affärsmöjligheter.

Med finansiering från Kunskaps- och kompetensstiftelsen (KK-stiftelsen) samarbetar forskare från GIH i projektet med företagen Coop, IKEA, Skandia, Skanska samt med de enskilda organisationerna Konsumentföreningen Stockholm (KFS) och Kronprinsessparets stiftelse, Generation PEP, i ett försök att motverka trenden med psykisk ohälsa bland skolungdomar.

Det finns en akut brist på kunskap om förhållandet mellan fysisk aktivitet, stillasittande samt kost och hälsosamma hjärnfunktioner, vilket förhindrar utveckling av verksamheter såväl som varor och tjänster. Vidare finns det kunskapsluckor när det gäller hur skolor kan arrangera regelbunden och meningsfull fysisk aktivitet för skolbarn under skoldagen.

Kärnfrågan undersöktes i tre delprojekt. Forskning inom delprojekt 1 tog fram ny kunskap om de övergripande relationerna mellan fysiska aktivitetsmönster, kost och hälsosamma hjärnfunktioner bland skolungdomar. Denna kunskap fungerade som underlag för studier inom delprojekt 2 av akuta neurofysiologiska och psykologiska effekter av viss fysisk aktivitet samt frukost på fysiologiska mekanismer av betydelse för hälsosamma hjärnfunktioner. I delprojekt 3 genomfördes kvalitativa undersökningar kring hur fysisk aktivitet bland ungdomar sker under en vanlig skoldag och hur skolan jobbar med uppdraget att erbjuda fysisk aktivitet inom ramen av skoldagen.

Två ytterligare projekt togs fram utifrån de första tre delprojekten. Under våren 2021 kontaktades deltagarna i delprojekt 1 igen för att undersöka hur COVID-19 pandemin som bröt ut under början av år 2020 hade påverkat deras fysisk aktivitet, livsstil och hälsa. Resultat från delprojekten användes även som utgångspunkt för insatser i skolor, där skolpersonal i samarbete med forskare, affärspartners och enskilda organisationer, samverkar för att finna vägar till regelbunden och meningsfull fysisk aktivitet under skoldagen i syfte att främja hälsosamma hjärnfunktioner, se mer om det projektet här. Abstract [en]
The research project "Physical Activity for Healthy Brain Functions in School Youth", abbreviated "Brain health in school", aims to investigate the relationships between habitual physical activity and healthy brain functions (i.e. good cognitive functions and good mental health), the acute neurophysiological effects of physical activity, sedentary and diet, and how schools can engage in physical activity for the promotion of healthy brain functions.

The core question of the project is: What is the relationship between physical activity, sedentary lifestyle, diet, and healthy brain functions among school youth, and how can physical activity for the promotion of brain health be implemented in a long-term sustainable way in schools?

The background to the research question is grounded in the growing mental ill health and reduced school results among children and adolescents in Sweden. Mental ill health leads not only increased suffering, but also poorer ability to study and work and a higher risk of future diseases. Impaired cognitive abilities can affect the ability to complete academic education and therefore pose a threat to Swedish competitiveness in a global perspective. The project "Brain Health in School" captures several of the global goals for sustainable development in Agenda 2030 and contributes to turning the goals into business opportunities.

With funding from the Knowledge Foundation (KK-stiftelsen), researchers from GIH are collaborating on the project with the companies Coop, IKEA, Skandia, Skanska and with the non-governmental organisations Konsumentföreningen Stockholm (KFS) and The Crown Princess Couple's Foundation, Generation PEP, in an attempt to counteract the trend of mental ill health among school youth.

There is an acute lack of knowledge about the relationship between physical activity, sedentary lifestyle, as well as diet and healthy brain functions, which prevents the development of businesses as well as goods and services. Furthermore, there are knowledge gaps in how schools can arrange regular and meaningful physical activity for school youth during the school day.

The core question was investigated in three sub-projects. Research within subproject 1 produced new knowledge about the overall relationships between physical activity patterns, diet and healthy brain functions among school youth. This knowledge served as a basis for studies within subproject 2 of acute neurophysiological and psychological effects of certain physical activity as well as diet on physiological mechanisms of importance for healthy brain functions. In sub-project 3, qualitative studies were carried out on how physical activity among adolescents takes place during a normal school day and how the schools work with the goals to offer physical activity within the framework of the school day.

Two additional projects were developed based on the first three sub-projects. In the spring of 2021, the participants in sub-project 1 were contacted again to investigate how the COVID-19 pandemic that broke out in early 2020 had affected their physical activity, lifestyle and health. Abstract [en]

PURPOSE: Mental health problems are increasingly prevalent during adolescence. Nutritional factors, particularly antioxidants, are of interest due to their potential to reduce oxidative stress and inflammation linked to mental health issues. However, the relationship between dietary antioxidants and adolescent mental health remains unclear. This study aimed to investigate this association in Swedish adolescents and explore potential gender differences.

METHODS: Cross-sectional data were gathered among Swedish boys and girls aged 13-14 years (n = 1139). Participants reported their dietary intake using a detailed web-based method and mental health outcomes, including anxiety, psychosomatic symptoms, and health-related quality of life (HRQoL) using self-report scales. Multiple linear regression analysis, adjusted for confounders, was used to investigate the associations between mental health outcomes and tertiles of dietary intake of vitamin C, E and β-carotene.

RESULTS: Adolescents in the highest tertile of β-carotene intake reported lower anxiety (β=-1.23, 95% CI=-2.34, -0.12), fewer psychosomatic symptoms (β=-0.91, 95% CI=-1.69, -0.13), and better HRQoL (β = 0.89, 95% CI = 0.11, 1.68). Similarly, higher vitamin C intake was associated with fewer psychosomatic problems (β=-1.00, 95% CI=-1.79, -0.21). Vitamin E intake showed no associations. Significant gender interactions were observed on the multiplicative scale analysis and limited to the middle tertiles of β-carotene and vitamin C for anxiety and psychosomatic symptoms, respectively.

CONCLUSION: Our findings underscore the potential role of dietary antioxidants, particularly β-carotene and vitamin C, in adolescent mental health. Further research including diverse populations and employing prospective designs could deepen the understanding and inform public health interventions.

Abstract [en]

AIM: Investigate the longitudinal relationship between physical activity, organised physical activity, fitness, screen time and academic performance among Swedish adolescents.

METHODS: Data from 1139 adolescents at age 13, included vigorous physical activity (accelerometry), fitness (submaximal ergometer test), screen time and organised physical activity participation (self-reported) and academic performance (math and Swedish grades at ages 13 and 16 from registry). Academic performance at age 16 was categorised as A-D (higher grades or pass) or E, F (fail or at-risk of failing). Multilevel logistic regression models were used to examine the relationships while adjusting for gender, parental education, parental country of birth and academic performance at age 13.

RESULTS: Higher fitness at age 13 was associated with increased odds of receiving A-D at age 16 (OR: 1.04 per mL, 99% CI 1.00, 1.07, p = 0.003). High screen time during weekdays was associated with reduced odds (OR: 0.40, 99% CI 0.20, 0.81, p = 0.001) compared to low screen time.

CONCLUSION: Academic performance at the end of compulsory school (age 16) was related to fitness and screen time 3 years earlier. These findings create a paradigm for future randomised controlled trials to explore how influencing these factors might affect academic performance.

Abstract [en]

Tackling inequities and inequalities related to adolescents' engagement in physical activity and sport is highlighted in recent research. However, rarely addressed are the issues of intersecting hierarchal systems of power that interlocks on both micro and macro levels, affecting how physical activity patterns express social memberships across several contexts. Therefore, this study aimed to explore how adolescents' physical activity during leisure and school hours expressed intersecting social positions and institutional school segregation, using a critical intersectionality approach. Nested in a larger study, a cross-sectional design was applied, including a total of 1139 adolescents in grade 7 (age 13-14) from Swedish school settings. Data was retrieved from surveys, accelerometers, and national registries. Descriptive statistics and stratified analyses were performed to examine levels of moderate-to-vigorous physical activity (MVPA) during school and leisure hours connected to intersecting social positions and school segregation contexts. The statistical analysis indicated that MVPA, to varying extents, expressed both individual social position and school segregation across the sub-groups explored. Inequalities in MVPA were closer connected to intercategorical gendered social positions in school contexts, rather than during leisure hours. However, there were significant intracategorical differences among girls during leisure hours across segregated contexts. Our results suggest that the power structures of school segregation together with educational and sport logics intertwined seem to 'disadvantage' especially girls with a non-ethnically Swedish background. Yet, it also highlights a need to problematize 'the absent minority girl' in equity focused physical activity research. This study emphasizes how an intersectionality approach can shed light on social (dis)advantages connected to adolescents' MVPA from two poles of power, contributing to a more comprehensive and critical understanding of social inequities in physical activity and health research.

Abstract [en]

Aim: Insufficient physical activity among adolescents, combined with poorer well-being and mental health, are worrying societal problems. Adolescents with ADHD encounter daily challenges and are more prone to experiencing mental and physical health issues compared to their peers. The aim was to investigate associations between moderate-to-vigorous physical activity, sedentary time, screen-time, psychosomatic problems, and ADHD symptoms in adolescents. Method: Data came from a cross-sectional study. Participants (N = 1139) were seventh-grade students who answered questionnaires on screen-time, psychosomatic problems, hyperactivity, and ADHD and were assigned an accelerometer, worn for seven consecutive days. Data were analyzed using linear regression. Results: A negative association was found between moderate to vigorous physical activity and psychosomatic problems in the entire sample. Moderate-to-vigorous physical activity was more strongly associated with psychosomatic problems for adolescents with self-reported ADHD symptoms (B = -0.14 (-0.24, -0.04)) than adolescents without ADHD symptoms (B: -0.03 (-0.05, -0.01)). Conclusions: Moderate-to-vigorous physical activity, sedentary time, and screen-time were all significantly associated with psychosomatic problems among all participants. Moderate-to-vigorous physical activity showed a stronger association with psychosomatic problems among participants with ADHD. This indicates that this group may be particularly responsive to interventions attempting to promote mental health through physical activity.

Abstract [en]

Numerous scholars within sport and health sciences have emphasized the critical role of the school environment in shaping young people’s physically active lives. While this PhD project addresses this specific dimension, the inquiry is also situated within wider academic discourses (ways of thinking, speaking, and producing knowledge within scholarly communities) concerning adolescents’ physical activity in school settings. School contexts are often narrated as ‘ideal’ for promoting young people’s physically active lives, with physical activity research carrying broader assumptions where the goal often is to prescribe movement, to get students to be more active. However, such assumptions rest on a set of foundational commitments: ontological (concerning the nature of reality), epistemological (concerning how we come to know things), and axiological (concerning values). Together, these shape our understanding of what physical activity is, how it should be studied, and why it matters. Similarly, the ways in which school contexts are portrayed – including the roles of students and staff within these spaces – are shaped differently across academic discourses, depending on the underlying commitments of interventionist, pedagogical, and critical research traditions.  As the school context emerges not merely as a backdrop but as a lived and negotiated space, whole-school and comprehensive strategies have gained traction among global agencies and physical activity scholars. These strategies emphasize holistic initiatives that span from individual to systemic levels, integrating curriculum, school culture, policies, student and staff sub-groups, and community engagement. Ecological frameworks and theories are often advocated to guide such strategies, highlighting the interplay between individual actions and broader social and environmental influences. However, the way ecological frameworks are applied varies depending on the academic discourse in which the research is situated, as different discourses engage with them from distinct perspectives. In particular, engagement with the concept of agency varies significantly. Despite their widespread use, the frameworks often lack critical perspectives, with issues such as power dynamics and equity rarely being foregrounded. These differences and omissions need to be critically examined for current and future research initiatives, particularly as scholars and agencies increasingly advocate for inter- and transdisciplinary collaborations.

The current PhD project adopts a meta-perspective, analyzing patterns in how school contexts are articulated across academic discourses. Four individual articles – each situated within different research paradigms, perspectives, or discourses – serve as illustrations of what can be learned from their juxtaposition, building on prior research. The individual articles are founded on three sub-studies: an ethnographic study combined with a social network survey, a cross-sectional study using data from surveys, accelerometers, and national registries, and a systematic review assessing school context measurements. This methodological combination is both novel and cross-paradigmatic when studying physical activity during the school day for young people in secondary school.

An analysis was conducted to explore how school contexts are constructed in relation to adolescents’ physical activity, using the included articles as the basis for inquiry. Through an examination using three analytical components – diverse goals, multiple strategies, and varied interest groups – three rhetorical patterns were generated. The patterns frame school contexts as (in)effective backdrops, as spaces of student and teacher meaning-making, and as oppressive and empowering environments. The articles were further mapped against four complex elements that shape school environments: organizational subgroups, formal structures, sociocultural aspects, and broader networks. The findings suggest that these elements are deeply intertwined across the articles, with varying degrees of emphasis and integration, providing a more nuanced understanding of the rhetorical constructions and contextual dynamics that inform physical activity research within school settings. The three patterns and how the elements are addressed adhere in different ways to interventionist, pedagogical, and critical research.

Concluding with a discussion on the potential spaces for rethinking physical activity research in school contexts, the exploration focused on where boundaries between academic discourses become more permeable – sites where interventionist, pedagogical, and critical research can coexist not through resolution, but in productive tension. Rather than seeking synthesis, the aim was to hold the perspectives in juxtaposition, allowing their distinct logics and values to interact and inform one another. This opens up opportunities for multidimensional thinking, addressing effectiveness, meaningfulness, and equity. The conceptualization of agency and participatory methodologies is central, which are well-established yet increasingly relevant concepts that offer spirited passageways for advancing physical activity research in school contexts. By attending to the relational and spatiotemporal dimensions of agency, and foregrounding the voices of students, staff, and school communities, the thesis is not thought of as a solution. Rather, it is positioned as a strategic entry point for inter- and transdisciplinary collaboration – an invitation to play with and imagine futures shaped through dialogue, reflexivity, and shared inquiry.

Recent research has urged us to rethink how we promote ‘more’ physical activity in school settings. However, schools are also arenas of competing paradigmatic interests among scientific communities. This raises the question: how are school contexts for adolescents’ physical activity conceptualized and articulated as a response to the well-being agenda across interventionist, pedagogical, and critical research perspectives?

How Physical Activity and School Settings Are Framed – School contexts are often narrated as ‘ideal’ for promoting young people’s physically active lives, with physical activity research often carrying broader assumptions where (often) the goal is to prescribe movement, to get students to be more active. However, such assumptions are underpinned by a set of onto- episteme-axiological commitments – about what physical activity is, how it should be studied, and why it matters. ​​​Schools are increasingly seen as dynamic spaces for promoting physical activity through whole-school and comprehensive strategies that integrate curriculum, culture, policy, and community. While ecological frameworks are commonly used to guide these efforts, their application varies across academic discourses and often overlooks critical issues like power and equity. Addressing these gaps is vital as the field moves toward more interdisciplinary and inclusive approaches. ​

Ways of Knowing, Ways of Doing – ​This project adopts a meta-perspective, and methodologically, it combines ethnography, social network analysis, cross-sectional data, and systematic review – offering a novel, cross-paradigmatic approach to studying adolescents’ physical activity in Swedish secondary schools.​​Three rhetorical patterns were identified in how school contexts are constructed in relation to adolescents’ physical activity: (i) as an (in)effective backdrop focused on behavior change, (ii) as meaning-centered spaces shaped by student-teacher interactions, and (iii) as sites of either oppression or empowerment. These patterns reflect differing priorities – highlighting the complex and contested nature of promoting physical activity in schools.

Future Pathways Forward?​ – This project identifies spaces and offer pathways for collaborative, multidimensional research grounded in effectiveness, meaningfulness, and equity – foregrounding agency and co-creation through participatory methodologies, in reframing of physical activity research.

Abstract [en]

AIM: Evidence on physical activity (PA), sedentary time, and fitness in adolescents with impairments has been limited. We aimed to compare outcomes in Swedish adolescents with and without impairments and between impairment types.

METHODS: This cross-sectional study, from September to December 2019, comprised of adolescents from 34 mainstream schools within 3 h' drive of Stockholm, Sweden. Parents reported impairment status. PA and sedentary time were measured with accelerometers during school and leisure time on weekdays and weekends. Fitness was estimated using the Ekblom-Bak submaximal cycle test, sports participation was self-reported and multilevel mixed models were used for analyses.

RESULTS: We enrolled 972 adolescents (51% girls), with a mean age of 13.4 ± 0.3 years. Just under a third (31%) had impairments. Adolescents with impairments showed lower PA levels, less adherence to recommendations, lower fitness and less participation in organised sports than those without impairments. Those with learning or visual impairments engaged in less vigorous activity and the former had lower fitness levels.

CONCLUSION: Adolescents with impairments were less physically active, more sedentary and had lower fitness than peers without impairments. This emphasises the need for equitable opportunities for PA, to support long-term health and well-being in adolescents with impairments.

Abstract [en]

It remains unknown whether dietary nitrate and breakfast may enhance working memory (WM) performance by augmenting physiological mechanisms and subjective psychological well-being. We performed a 3-arm randomised within-subject crossover study, with pretest-posttest comparisons, to test whether nitrate consumption via breakfast with a beetroot juice shot or regular breakfast compared to no breakfast improved WM (measured with n-back tests) and cognitive task-related changes in prefrontal cortical haemodynamic response (oxygenated- and deoxygenated-haemoglobin derived from functional near-infrared spectroscopy). In addition, effects on peripheral vascular function and self-reported psychological factors were assessed. In 60 adolescents (13-15 years old; 66% girls), WM improved in all conditions, with no intervention effects. Intervention effects were seen for oxygenated-haemoglobin changes, such that it increased after the breakfast with a nitrate shot during the WM tests and decreased after the regular breakfast. Thus, different neurophysiological mechanisms may be at play to preserve WM in adolescents depending on their breakfast composition. The trial was registered in the ISRCTN registry (ISRCTN16596056) on 21/02/2022.

Abstract [en]

Earlier research proposes that a school’s social environment among staff functions as an important facilitator for physical activity policy/curriculum dissemination, such as support among colleagues, physical activity ideas shared between teachers, as well as having individuals as ‘agents of change’. It is often proclaimed that physical education teachers inhabit these ‘agentic’ roles concerning physical activity and health promotion targeting youth during the school day. Despite this, little seems to have been researched concerning the agentic roles of physical educators and other school staff and their social networks within the school organization. Consequently, this study aims to explore relations among school staff regarding daily physical activity promotion in a Swedish secondary school from a social network perspective.  

This case study applied a convergent mixed method design with an emergent approach, where the qualitative ethnographic data and quantitative survey data were collected around the same time. Data was collected during one full school year from school staff at one secondary school (students aged 12-16 years, grades 7-9) in a larger urban area in Sweden. The sample included all staff within the school that interacts with secondary students, which involves school management, staff and teachers who are responsible for secondary students at some point during the school day, involved in student health or are engaged in physical activity, as well as physical education and health teachers. Our empirical material consists of field notes (180 hours of fieldwork), transcriptions from interviews (12 staff), as well as survey nomination data (23 participants). Qualitative data has been analyzed through a reflexive thematic analysis, and survey data through a social network analysis, and then further combined in an integrated analysis.  

Preliminary results of the current study show that the school staff’s social networks are characterized in different ways within close everyday-discussion colleague networks, peer networks concerning physical activity, and advisory networks regarding physical activity. In the everyday discussions among staff on topics regarding secondary students, physical education teachers are positioned as isolated islands in the networks. While physical education teachers hold a somewhat key role relating to discussions about physical activity, these discussions also go through other, and sometimes more influential, agents. Conflictingly, the physical education teachers still picture themselves as physical activity ambassadors for students’ daily physical activity. This picture is also shared by other school staff. At this school, the results shed light on important issues concerning how the social (network) context among school staff helps promote and inhibit physical activities for the students.

Gymnastik- och idrottshögskolan (GIH) har sedan 2017 arbetat i en serie forskningsstudier finansierade av Stiftelsen för kunskaps- och kompetensutveckling, KKS. Studierna har fokuserat på både barn, vuxna och äldre. Sedan 2021 finns vid GIH ett forskningscentrum med fokus på fysisk aktivitet och hjärnhälsa, det vill säga goda kognitiva funktioner och god psykisk hälsa, kallat E-PABS (Center of Excellence in Physical Activity, Healthy Brain Functions and Sustainability). Forskningen inom E-PABS täcker många ämnesområden och innefattar epidemiologi, beteendeförändring, kognitiv neurovetenskap, fysiologi och molekylär biokemi.

Denna rapport bygger på studier inom projektet ”Fysisk aktivitet och hälsosamma hjärnfunktioner hos skolungdomar”, som genomfördes under 2017–2023. Projektet hade tre delar, en epidemiologisk, en fysiologisk och en etnografisk del som tillsammans syftade till att svara på frågorna ”Vilket är sambandet mellan fysisk aktivitet, stillasittande beteende, kost och hälsosamma hjärnfunktioner hos barn och ungdomar, och hur kan fysisk aktivitet som främjar sunda hjärnfunktioner långsiktigt hållbart implementeras i skolor?”
